This designation places them among a select number of primary care practices in Michigan that are improving healthcare quality and lowering costs through a coordinated, team focus on their patients’ health and wellness.
As a patient-centered medical home, we give our patients access to many great services. A member of your care team can answer questions about urgent health needs 24 hours a day, seven days a week.
