Medical Records

Your medical record is confidential. We will not talk about your condition with anyone without your permission. If you wish to release information from your medical records, complete a Release of Information form and return to our office.

Hospitalization

Our doctors and advanced practice providers only see patients at our office. If you need to go to the hospital, you will be cared for by a doctor at that hospital. If you are admitted to Bronson, another doctor will take care of you and keep your Bronson provider aware of your illness or injury. After you leave the hospital, you will see your provider for follow-up care.
