How to Request Your Medical Record

You have three ways to request your medical records.

Download Your Medical Record

Login to your Bronson MyChart account and select Download My Record under the Health tab. Select the option that matches the information you need.

Share Your Medical Record With Other Health Care Providers

You can share one-time, limited access to your health record with just about anyone involved in your care. Whether your provider uses Epic or not, you can let them see your health record summary, medications, allergies, immunizations and health issues. Visit your Bronson MyChart account to share your record. Don’t have a Bronson MyChart account? To experience connected care, request your code now.

Request a Release of Information

Fill out the Release of Information. Be sure to sign the form. Unsigned requests cannot be processed. Your request will be fulfilled within 30 working days.

Need to Request an Amendment to Your Medical Record?

You have the right to request that we amend your Protected Health Information maintained in your medical record file or billing records. If you desire to amend your records, please mail or fax an amendment request form from Health Information Management (Medical Records). We will comply with your request unless we believe that the information that would be amended is accurate and complete or other special circumstances apply.
