Blood Clot Management
- Assessment and treatment of blood clots in the home
- Education regarding blood thinner medications and other preventative therapies
- Education regarding signs and symptoms of blood clots and next care steps
Children's Healthcare
- Skilled nursing evaluation and treatment of premature infants and adolescents, including weight checks, failure to thrive, malnutrition, cardiac care, chronic disease management, infections and infusion therapies
- Physical, occupational and speech therapy for the treatment of premature infants and adolescents for developmental milestones
Chronic Disease Management
- Services to improve the quality of life through disease progression education and medication management, of conditions such as heart failure, chronic obstructive pulmonary disease (COPD), diabetes and wounds
Diabetes Management
- Education on healthy lifestyle and nutrition choices as well as information on the signs and symptoms of high and low blood sugar levels. Assistance with blood glucose testing and education regarding antihyperglycemics, including insulin and oral or injectable medications
Fall Prevention
- Home safety evaluation for fall risk and prevention strategies
- Education on exercises, balance and safety strategies to improve safety at home
Heart/Cardiac Care
- Home safety evaluation for fall risk and prevention strategies
- Specialized program for acute and chronic heart failure patients to manage health at home, which lowers readmission to the hospital due to heart failure
- Cardiac treatment and education on post-surgical conditions, heart attacks, stents, irregular heartbeat (A-Fib) and other cardiac conditions
- Education on a heart-healthy diet and lifestyle and nutrition choices
In-Home Health Management
- Skilled nursing, certified physical therapy, occupational therapy, home health aide and medical social work services for personal health care needs in the home
Intravaneous (IV) Therapy
- Nursing services providing infusion care, such as antibiotics, total parenteral nutrition (TPN), monoclonal antibodies and hydration for in-home management of acute and chronic disease processes
- Dressing changes to veinous access sites and lab draws
LSVT - BIG
- Certified physical therapy and occupational therapy for the treatment of Parkinson's disease
Lymphedema Support
- Skilled nursing services, certified physical therapy and occupational therapy for treatment of edema and accompanying wound care
Medical Social Work Support
- Assistance with community resources and long-term planning
Medication Teaching
- Skilled nurses and therapists provide education on medications, including indications, schedules and side effects
Nutrition Support
- Total parenteral nutrition (TPN), tube feeding services and education in collaboration with area pharmacies, registered dieticians and providers
Orthopedic & Joint Replacement Care
- Skilled nursing services, certified physical therapy and occupational therapy to manage post-acute joint replacement care
- Review of pain medication after surgery as well as the development of home exercise programs and evaluation of living environments for safe and successful return to community activities
Post-Surgery & Incision Care
- Skilled nursing for infection prevention while caring for your incision at home
- Education regarding wound care and wound care products
- Education regarding the prevention of post-surgical complications
Rehabilitation Services
- Physical therapy, occupational therapy and speech/language in-home therapy for short-term evaluation and treatment
Respiratory & Pneumonia Care
- Skilled nursing services to evaluate and promote lung/respiratory health
- Education regarding respiratory medications and devices including inhalers, oxygen and breathing treatments
Stroke & Neurological Care
- Skilled nursing services to evaluate and promote safe access and mobility in the home. Implementation of a home program including individualized exercises, medical device recommendations, medication management and speech/language therapy
Vestibular Benign Paroxysmal Positional Vertigo (BPPV)
- Skilled physical therapy for the treatment of positional vertigo with education for continued home exercise programs
Wound & Ostomy Care
- Skilled nursing services to assist and educate the patient, family or caregivers about at-home independent wound and ostomy care
