Financial Assistance Application
Bronson offers financial assistance for patients who need or have already received emergency or medically necessary care but cannot afford to pay. Patients who do not have insurance or are insured and have balances after their insurance pays may be eligible for financial assistance. You may be considered if:
- Your family income is under 350% of the current federal poverty level
- You live in Michigan, Illinois, Indiana or Ohio
- You are not eligible for Medicaid
All financial assistance documents (the Financial Assistance Application, Financial Assistance Policy, and Financial Assistance Plain Language Policy Summary) can be found in the menu to the left in English, Spanish, Arabic and Burmese.
To apply, please review and submit the application with needed documents, or contact our Bronson Billing Department: (269) 341-6117 or (800) 699-6117. Office hours are Monday through Friday 8:30 a.m. to 5:30 p.m.
If you need assistance filling out an application, Bronson Financial Counselors are available on-site at each hospital to assist you. General office hours are Monday through Friday 8:30 a.m. to 4:30 p.m.
